NSPCC workers did not piece together a string of contacts with Asad Niazi or follow up their first referral to Kingston Council.
Although the DHR says professional standards set by the NSPCC were all met, the charity has now put in place new protocols around domestic violence, with referrals given the highest priority.

The review also says a worker at Victim Support, after a call from Ms Cruz, did not escalate the call to a manager.
Victim Support divisional manager Anna Twomlow said: “Our policy and process has changed.”
