Kingston Hospital staff suffered accidents and abuse from patients 356 times last year.
Between April 2013 and March this year, 113 instances of abuse by patients were logged - including 53 physical assaults.
There was also one instance of staff-on-patient assault reported, and one of staff-on-staff.
A hospital spokeswoman said both these incidents were "very minor", and were dealt with internally without police involvement.
Incidents involving sharp instruments like needles rose from 85 to 98, prompting a renewed safety drive and extra guidance for workers.
Staff slipped, tripped or fell 40 times, according to the hospital's 2013-14 health and safety report.
A third of the falls occurred on wards.
The report also said 784 security incidents were logged, including 36 thefts and 134 missing people.
